What happens if my care needs change?
At Brunelcare we strive to ensure all our customers are supported to make the most of later life, whether that’s at our sheltered housing sites, in their own homes or in one of our care homes.

The Brunelcare pathway
At Brunelcare we provide a range of services to support you through all stages of care, from our independent sheltered housing to care on-site at our extra care housing, all the way to full-time 24/7 nursing care in one of our specialist care homes.
Whatever your care needs, we can support you through the journey to help ease any stress as your situation changes.
If you are currently using one of our services and would like to find out more about your options with us as your care needs progress, please contact your site officer or manager who will be able to explain the different options available to you.
When care needs change within our care homes
When your loved one moves into one of our care homes, we will create a personalised care plan tailored to them to make sure their needs are being met, and conduct ongoing reviews to identify if these needs have changed over time. We’ve written this brief guide to help you understand what to expect if your loved ones needs have changed, and how we will support you through this.
If your loved one’s needs change:
- Residents will be fully reviewed by their registered GP to understand if their needs have changed and where they may need extra support.
- Our dedicated care and nursing team will deal with any referrals to specialist services that relate to any change in needs, whether that’s a referral to mental health care or dementia support services.
- We will consider a change of community or room in certain circumstances to ensure your loved one is receiving the highest level of care possible, and we will support them through this change in environment. For example, we may move your loved one to a more visible room with enhanced supervision for additional safety if they are at a high risk of falls.
- If your loved ones health needs are complex, our team will complete a continuing healthcare referral, and if they are eligible you may receive funding support from the NHS as part of this. You can rest assured knowing we will complete this referral for you to avoid any additional stress.
- If all other care interventions are exhausted, we may need to make a request to the funding authority or family of the resident for extra care resources. Our care team will work hard to try all other options before this is considered, and you will be involved in decision making throughout the entire process to keep you informed.
- We will keep families and lasting power of attorneys involved in all discussions during this process. We understand this can be a challenging time, so we will aim to ensure you feel supported and informed at all times.
