Brunelcare’s Somerset Community Services team home care service is designed to support individuals within their own home and help maintain their independence.
There are three home care services we currently provide within Somerset:
- The support within your home we provide ranges from support with domestic tasks and shopping to support with personal care routines, meals and medication
- We also provide ‘respite care’ within people’s homes. This is provided if someone’s main carer requires respite from providing 24/7 care.
- There is also a ‘reablement’ service we help provide, called ‘Home First’. Available only in Somerset, from Brunelcare’s Somerset Community Services (home care) team who provide short term care at home to people discharged from Musgrove Park Hospital and Yeovil District Hospital, helping these individuals home faster and rehabilitating at home rather than hospital. For Home First service you would be assigned this care by a hospital discharge team as your eligibility would be established by Somerset County Council NHS system, working in collaboration with Brunelcare.
How does payment for the service work?
These services can be funded privately, via your personal budget, or through Somerset County Council.
